COVID-19 Vaccine

Happening Now: Updated (Bivalent) Booster now available to those at least two months after 2nd primary dose or last booster.

Children 6 months and older are recommended to get vaccinated, and boosters are now available as of December 9, 2022.

Anyone who received their first booster vaccine at least four months ago are now able to receive a second booster. Please book an appointment to get your second booster and protect yourself against COVID-19.

Heart City Health is now offering COVID-19 vaccines through scheduled patient appointments. If you are not a Heart City Health patient, please call 574.293.0052 to become a new patient.

We are still following all State guidelines, including the rules regarding the phases of distribution and social distancing practices.

Pfizer and Moderna COVID-19 vaccines are available and require two doses.

NOTE: Pfizer is now for those ages 6 months and older. A parent or guardian must accompany the child if under 18 years old. Moderna is available for 6 months to 18 years and older and requires 2 doses.

Mobile COVID-19 Vaccine Clinics

Heart City Health partners with multiple organizations, including the City of Elkhart, to make the COVID-19 vaccine available to all Indiana residents. Mobile clinics below have the option to schedule an appointment or walk-in.

COVID-19 Testing

Please call your provider at Heart City Health, rather than going into the office if you think you have symptoms of COVID-19.

You can schedule an appointment for free testing with the State of Indiana online.

If you are a Heart City Health patient, we will test for COVID-19 if it is ordered as part of your scheduled medical appointment and considered necessary.

The CARES Act requires us to post the cash price of COVID-19 tests on our website.

Heart City Health administers COVID-19 tests and must bill through insurance (copays do not apply). If you do not have insurance, you will not be charged for the test. Heart City Health is required to disclose that we incur a $100 charge for each COVID-19 test. We will not turn patients away for not having insurance.

Heart City Health COVID-19 testing criteria is as follows:

  • Symptoms of COVID-19
  • Exposure to someone who tested positive
  • A written order from your doctor
  • Have previously tested positive (14 days prior) and you need to be retested.
