Heart City Health Partners with City of Elkhart and Offers Free COVID-19 Vaccine at Elkhart Jazz Festival

ELKHART, IND. – Heart City Health has partnered with the City of Elkhart to provide a free COVID-19 vaccine to adults ages 18 and older at the Elkhart Jazz Festival.

“We are appreciative of the City of Elkhart for this opportunity, let alone at the Elkhart Jazz Festival, which reaches a national audience,” says LaLaesha Black, Chief Operating Officer at Heart City Health. “This latest vaccine clinic is just one of many we are providing throughout Elkhart County. Our objective is to provide services to the community, out in the community, not only in our office locations.”

The vaccine clinic is Saturday, June 19, 2021, from 12:00 p.m. to 8:00 p.m. at the Civic Plaza Downtown, 350 S. Main Street in Elkhart.

Walk-ins are welcome and no appointment is needed. The Johnson & Johnson one-dose vaccine will be administered to those 18 years and older. A second dose is not required. To get your vaccine, you will need to provide:

  • Name
  • Date of Birth
  • Photo ID

You will not need to provide a Social Security number, proof of citizenship or legal status, or insurance.
