Heart City Health Offers Free COVID-19 Vaccine at Faith Mission of Elkhart
ELKHART, IND. – Heart City Health partnered with the Faith Mission of Elkhart, Minority Health Coalition of Elkhart County, and Saving Grace Advocacy to spread the word on a free COVID-19 pop-up clinic. The clinic is scheduled for Saturday, May 22, 2021, from 10:00 a.m. to 1:00 p.m. at the Faith Mission of Elkhart, located at 801 Benham Ave., Elkhart, IN 46516.
Heart City Health recognizes the need for convenience as some community members do not have access to transportation or the internet to schedule an appointment.
All adults in Indiana (18+ years old) are eligible for the COVID-19 vaccine. To get your vaccine, you will need to provide:
- Name
- Date of Birth
- Photo ID
You will not need to provide a Social Security number, proof of citizenship or legal status, or insurance.
200 doses are available first-come, first-served. Walk-ins are available and no appointment is necessary. The Johnson & Johnson one-dose vaccine will be administered, so a second appointment is not needed. If you have questions regarding COVID-19 vaccines, please visit https://ourshot.in.gov.
