Heart City Health Expands Integrated Behavioral Health Support Through Collaborative Care Model and NeuroFlow Tools

Elkhart, IN – Heart City Health is continuing to strengthen whole-person care for patients by offering integrated behavioral health support through its Collaborative Care Model, an evidence-based approach that connects mental and physical health services within primary care. Mental and physical health are closely connected, and Heart City Health is committed to meeting patients where they are by providing care that supports both. Through Collaborative Care, patients work with a dedicated care team that includes their primary care provider and a behavioral health care manager, who partner together to support overall wellbeing.

Heart City Health has been operating under the Collaborative Care Model since October 2024, expanding access to coordinated mental health support within the primary care setting. As part of this program, Heart City Health began implementing NeuroFlow tools on October 30, 2025, and officially went live with NeuroFlow’s managed care support and the patient-facing wellbeing app Onward on December 12, 2025.

NeuroFlow supports care teams by helping track patient progress over time, while Onward provides patients with guided resources and simple digital check-ins that can be completed anytime between visits. Patients enrolled in Collaborative Care may use Onward to support concerns such as, stress and anxiety, sleep improvement, mood and symptom tracking, regular wellbeing check-ins, and staying connected to care between appointments.

Onward is available for patients ages 13 and older, in accordance with privacy and digital health guidelines.

“Collaborative Care gives patients an added layer of support that fits into their everyday lives,” said Ali Mahamat, Care Manager at Heart City Health. “It allows us to work alongside their primary care provider while giving patients tools they can use between visits to stay engaged, track progress, and take small steps that make a real difference over time.”

Access to Onward is available to patients who are enrolled in Heart City Health’s Collaborative Care program. Participation is voluntary, confidential, and designed to complement in-person care. Patients interested in learning more are encouraged to speak with their Heart City Health care team to see if Collaborative Care is the right fit.
