Responsibilities As part of the integrated health model of Heart City Health Center, the Licensed Clinical Social Worker provides integrated behavioral health services to patients currently receiving primary care services at HCHC. The Licensed Clinical Social Worker is the primary interface for patients to access services such as peer/family counseling and support, screening assessment, brief treatment, consultative services, and education. This position will also work directly with primary care providers in strengthening relationship between behavioral health and primary care services at Heart City Health Center.
