Outreach Manager

Responsibilities The Outreach Manager will organize and manage programs and initiatives that promote community wellness. This position requires the ability to identify general community needs as well as the ability to identify services that are necessary for special populations. The Outreach Manager will monitor the implementation of the program and assess progress to determine changes in the running and focus of the program. This position may work with public health offices, clinics, shelters, hospitals, universities, and in many other environments. While overseeing the public health initiatives, it is required that this position will be participating in the actions and gathering community feedback. Responsibilities As part of the integrated health model of Heart City Health Center, the Licensed Clinical Social Worker provides integrated behavioral health services to patients currently receiving primary care services at HCHC. The Licensed Clinical Social Worker is the primary interface for patients to access services such as peer/family counseling and support, screening assessment, brief treatment, consultative services, and education. This position will also work directly with primary care providers in strengthening relationship between behavioral health and primary care services at Heart City Health Center.
