Responsibilities

When our patients require expanded services or services that Heart City does not provide, the Case Manager – Referrals will ensure that the referrals are scheduled in a timely manner, assemble clinical, demographic, and insurance information to transmit to the specialist’s office, and assist in obtaining needed medical records to meet provider requirements. Responsibilities

Performs intake assessment which includes obtaining/reviewing prior medical records. Understands and acts upon orders to include laboratory testing, immunizations, TB skin testing and referral for preventive health needs as indicated per disease management protocols. Monitors patients frequently for changes in health status after initiation of a new medication, recent hospitalization, or change in health status. Follows-up with patients when barriers to referrals are identified. Routes calls to triage and facilitates a response to urgent telephone calls, requests or visits from assigned patients.

The Executive Assistant provides all aspects of assistance and support to the CEO and Executive Management team, as assigned. Maintains working relationships with coworkers in other departments. Qualifications

Associates degree in business related is required. Specialized training in secretarial practices or business administration preferred. Must attend all assigned meeting, including the board monthly meeting, outside of standard business hours. Computer knowledge required. Windows, Microsoft Office application and internet savvy. Prior experience in the administrative assistant field, preferably as an Administrative or Executive Secretary. Must present self in a professional manner at all times. Must have a driver’s license. Clinical or hospital experience preferred. Working on grant writing or applications beneficial to this position.
