Outreach Manager

Responsibilities The Outreach Manager will organize and manage programs and initiatives that promote community wellness. This position requires the ability to identify general community needs as well as the ability to identify services that are necessary for special populations. The Outreach Manager will monitor the implementation of the program and assess progress to determine changes in the running and focus of the program. This position may work with public health offices, clinics, shelters, hospitals, universities and in many other environments. While overseeing the public health initiatives, it is required that this position will be participating in the actions and gathering community feedback. Enabling better assessment of community needs and adjustment of outreach programs as needed. This position is also responsible for the full range of grant and contract opportunities and activities for the organization. These activities include grant writing and reporting with public agencies, private foundations and corporations. Specifically, this position is responsible for writing proposals for both unrestricted operating revenue and restricted projects and for submitting timely and accurate reports for all existing grant funded projects. Medical Assistant

The Licensed Clinical Social Worker is the primary interface for patients to access services such as peer/family counseling and support, screening assessment, brief treatment, consultative services, and education. This position will also work directly with primary care providers in strengthening relationship between behavioral health and primary care services at Heart City Health Center.
