Affordable Healthcare in Elkhart County

Established in 1993, Heart City Health is a 501(c)(3) non-profit Federally Qualified Health Center (FQHC) providing affordable, high quality medical, dental, and behavioral health care to residents of Elkhart County.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233 (g)-(n). This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

However, Heart City Health conducts all of its activities in an entirely nonpartisan manner. Heart City Health may acknowledge the elected officials who support us, but we do not endorse, support, or oppose candidates for public office, nor do we intervene in an election on behalf of or in opposition to any candidate of a political party.

Is Heart City Health a free clinic?

No, we are not a free clinic. We provide healthcare services for a fee. However, because we are a Federally Qualified Health Center, we are able to offer reduced rates to people without insurance as long as they qualify based on income. Everyone is welcome as a patient.

How much does it cost to visit Heart City Health?

Costs will vary based on your specific situation. We accept all insurance including Medicare, Medicaid, employer-based insurance, and commercial insurance plans. For anyone without health insurance, we offer services on a sliding-fee scale, which means you pay only what you can afford.

Who can make an appointment with Heart City Health?

Anyone and everyone. No matter who you are, you can seek help for your healthcare needs with us.

How can I obtain my medical records?

You can obtain your medical records by calling 574-293-0052 or you can register for Patient Portal on our website to access your personal health information online.

How do I know if the clinic is closed due to severe weather or some other reason?

To find out if there are delays or closings at any of our sites, visit us at Facebook/HeartCityHealthCenter.

How do I make an appointment?

You can call 574-293-0052 or simply visit one of our locations. Anyone who does not arrive before the appointment time may be asked to wait or reschedule. We are unable to guarantee your appointment if you are late.

Can I see a primary care provider without an appointment?

We do see walk-in patients, but our scheduled patients have priority. You can count on us to do our best to fit you in the schedule without an appointment, but you should be prepared to wait.

If you are a new patient, you should also be prepared to complete the necessary paperwork. You can access these documents through our Resources Page or give yourself enough time to complete them in our lobby when you visit.

How can I provide feedback on my experience at Heart City Health?
