Becoming a New Patient

It’s easy to become a new patient at Heart City Health Center! Contact our office at (574) 293-0052 to make an appointment. Our friendly Patient Financial Services (PFS) staff will guide you on what to bring to get started. Please arrive at least 15 minutes before your scheduled appointment to complete the paperwork. Bring your insurance cards and medication lists to each appointment.

Uninsured?

You don’t need health insurance to be a patient of Heart City Health Center. If you do not have insurance, we have SLIDING FEE SCALE (SFS) available to qualified individuals. This plan can significantly reduce the cost of your office visits.

Sliding Fee Scale (SFS)

Your fees at HCHC are based on your income if you don’t have insurance. As a new patient and every 6 months thereafter, we ask that you provide some basic information to qualify you for the sliding fee scale discount. You could pay as little as $15 for your visit! The SFS is based on federal guidelines and is updated every February. Those making up to 200% of poverty are eligible for the SFS. Those above this amount pay full fee, but even our full fee is still the best in town!

Applying for Assistance

To become eligible for the Sliding Fee Scale, you must complete an application with some basic information about your income and family size. You must be able to provide approved documentation to support your application. Please refer to our Resources FAQ for the exact list of documents required or contact our Patient Financial Services (PFS) staff at (574) 293-0052.

Payments

Payment is expected at the time of service for all cash paying patients. If you have insurance and are expected to pay a co-pay and/or deductible, that amount is due on the day of service.
For your convenience, we now accept credit and debit card payments.

Frequently Asked Questions

What do I need to do to become a patient?

Call (574) 293-0052 to schedule a medical appointment. Our Receptionist will ask you some questions and start the process for you to receive care. For a Dental appointment, call (574) 970-1937.

Are you a free clinic?

No, however, we will work to get you into a health care assistance program that is affordable. Our minimum visit fee is $15 and is based on income and family size. You may pay more depending on your specific financial situation. Any payment will be explained in advance of your appointment and our staff is happy to answer any questions you may have. Payment is expected at the time of service.

What if I don't have insurance?

If you do not have health insurance, our sliding fee scale (SFS) can help you save up to 80% on our services, based on your household income and family size. Our Patient Financial Services (PSF) staff will work with you to identify any assistance programs that might cover the costs of care.

What if I don't have any money?

We will work with you, but it is your responsibility to meet your financial obligations. Our sliding fee scale (SFS) provides a significant discount for the uninsured.

Do you accept Medicare & Medicaid – or HIP?

Yes. We accept Medicaid, Medicare and HIP (Healthy Indiana Plan). We will soon be accepting private insurance.

Who are the medical providers?

Our health center uses a variety of medical professionals including Pediatricians, Internists, Family Practitioners and Nurse Practitioners. We also have Dentists and Dental Hygienists here to serve you. To read more about our great team of professionals, visit "Our Team."

Does the Dental Clinic do dentures?

Yes, we are a comprehensive dental center. A payment arrangement for dentures is required.

Can you help with my prescriptions?

Yes. Heart City Health Center has a full retail pharmacy on-site. As a patient, you can get most prescriptions at a much discounted price. We can also help you get signed up for Medicine Assistance Programs (MAP) if you qualify. Through the MAP, you could get your prescriptions for only a small administrative fee ($4).
